Human subjects permissions
Several aspects of the studies I performed
for this portfolio required authorization by the Indiana University human
subjects committee. I prepared an
informed consent form (Appendix 3) on which students
could separately chose whether or not:

to take (and
allow me to use in this portfolio) the pre- and post-tests,
·
to allow me to
refer here to their exam answers, and
·
to allow me to
obtain from their admission files, and subsequently to use, their undergraduate
grade point averages and Graduate Record Exam scores.
These forms were administered by colleagues on the first day of class,
and not provided to me until after final course grades had been submitted to the
registrar.
